The Aftercare program is designed to offer individualised case management to AOD clients for up to 12-months in the community.  Support is client-centred and trauma informed. It often involves treatment planning, AOD counselling, evidenced-based treatment modalities (harm reduction, recovery focussed, CBT, DBT etc) psychosocial support, advocacy and support to engage with specialist services.  This is an intensive, creative and holistic program provided in an outreach capacity.

Our team and our clients need an Aftercare Case Worker who enjoys working with people.  You need to know your stuff when it comes to AOD and be committed to continual learning in this space.  As we know it’s ever changing.  We are a collaborative team who enjoy learning and supporting each other’s professional development.  We need someone who understands that recovery looks different for everyone and responds accordingly to their client’s individual goals.

Job Requirements

  • Minimum Diploma in AOD or a related health and community services Diploma with the 4 AOD core units
  • Experience in delivery of AOD/Mental Health client brief interventions and AOD Counselling and undertaking AOD Assessments
  • Experience in working with complex AOD/Mental Health clients presenting with challenging behaviors
  • A current VIC Drivers Licence
  • Valid VIC Employee Working with Children Check
  • Ability to provide proof of Eligibility to Work in Australia
  • You will be required to provide evidence of vaccine status dependent on requirements.
